Refer a Patient

At The Allergy, Asthma & Sinus Center, we want your experience with us to be so positive that you refer us to everyone you know who may be suffering from allergies, asthma or sinus issues. Here are some ways you can do just that.

1. Refer Online
   Refer a patient to our practice using our new online referral tool. Send the referral and any relevant medical records, labs, or other documents to our Scheduling Department – all online! Get the desktop referral shortcut here. (Just drag and drop our icon onto your desktop for quick access.)
   
   
2. Refer by Fax
   To refer a patient to any office via fax, complete our Fax Form and fax it to Scheduling at 865-584-6384.
   
   
3. Refer by Phone
   To refer a patient via phone, contact our Scheduling Department at 866-231-0701. One of our Patient Access Associates will be happy to help.
   
   
4. Refer Yourself!
   Yes, you can refer yourself to our practice without a referral or prior approval from your primary care or family physician. Schedule your own appointment online or contact our Scheduling Department at 866-231-0701 to make an appointment at the location nearest you.
   
   
5. Other Ways to Refer
   We can receive referrals through P2P, Direct Referrals (EMR to EMR), Order Facilitator, and other referral portals. For more information or to see if we can accommodate your referral method, call our Scheduling Department at 865-584-0962.
