We're located at 100 West 4th Street, Suite 250, across from the Cookeville Regional Medical Center.
Clinic hours are Mon., Wed., & Fri.: 8 am to 5 pm; Tues. & Thurs.: 8 am to 5:30 pm.
Who are we?

What do we treat?
